STEP #3  -  the Details

We accept most major insurances.  We will do our best to verify the details of your policy, such as out of pocket cost and visit limitations, but we recommend that you also contact your insurance company to verify these details.  If your insurance company requires authorization for physical therapy, our staff will take care of that!  Click here for additional insurance and financial information.   

All new clients should expect to spend about an hour with the therapist during their first appointment. During this time, the therapist will review the information from your completed forms, and also obtain information from you by asking additional questions and performing special evaluative tests. This information will be used to form a more complete picture of your condition, set goals and build your customized treatment program.


step #2  -  complete new patient forms

At your convenience, we ask that you print and complete our New Patient Forms.  The forms are in PDF format and require Adobe Reader.  Depending on the severity of your current condition and/or the extent of your medical history, they should take 10-30 minutes to complete.  

If you do not have access to a computer or printer, we would be happy to mail the forms to you, provided that there is enough time between the day that we speak with you and the date of your first appointment.

If there is not enough time for us to mail the forms to you and you do not have access to a printer, we ask that you come approximately 30 minutes early for your first appointment.

how to get started...

Thank you for choosing Valley Rehab and entrusting our staff to assist you with your health care needs.  This page will tell you everything you need to know about getting started with physical therapy.  

step #1  -  Call to schedule an appointment

On the phone we will review with you the things we need you to bring.  The list might include

  • Completed new patient forms (see Step #2)
  • Physical therapy prescription from your doctor (You may not have one if you are self-referred; we will let you know if your insurance does not cover P.T. without a prescription.)
  • Health insurance cards
  • Photo ID
  • List of all prescription and non-prescription medications, dosage and route of administration.

  • Shorts (if being treated for your leg/knee/hip)
  • Sleeveless shirt (if being treated for your shoulder/arm)

  • Child's walker, orthotics and/or braces
