CLIENT SELF-REFERRAL ~ DIRECT ACCESS to P.T.
Pennsylvania law now allows
qualified physical therapists to provide physical therapy treatment
to any client for up to 30 calendar days without
a prescription from a physician. After 30 calendar days, any client
who requires continued treatment must be seen by a physician to
obtain a prescription.
Valley Rehabilitation's
Susan Andrews is credentialed as a Direct Access physical therapist.
Please note that not all
insurance companies will honor or pay for Direct Access claims.
Please contact our office to find out if your insurance company
honors Direct Access claims or for additional information regarding
self-referral.

OUT OF POCKET COSTS
Please be aware that many
insurance policies are now written to include various out-of-pocket
costs to the members including co-payments,
co-insurance and/or deductibles. These costs are a contract
between you and your health insurance carrier, and you are legally
responsible to pay these amounts to your health care provider(s).
During your first
appointment, we will verify the specifics of your policy to
determine what out-of-pocket costs will apply to your treatments.
Co-payments must be paid on each date of service. We accept
cash and checks; we do not accept credit or debit cards.
