The following list is not comprehensive; if you do not see your insurance company listed below, please call us to see if your insurance company has included us in their provider network. If you have other questions regarding your coverage, please call us and we will do our best to verify the specifics of your policy.
We are committed to ensuring that everyone in need of physical therapy is able to receive the necessary care. If you do not have insurance, or your insurance does not cover outpatient physical therapy services, we can offer you a cash rate for your services. Please contact us for details.

Please be aware that many insurance policies are now written to include various out-of-pocket costs to the members including co-payments, co-insurance and/or deductibles. These costs are a contract between you and your health insurance carrier, and you are legally responsible to pay these amounts to your health care provider(s). We are unable to waive these fees, however we understand that these fees can be prohibitive. If you believe that your out of pocket costs may prevent you from receiving necessary treatment, please contact us about establishing a payment plan to spread your cost over a longer period of time.
Our staff will make every effort possible to verify the specifics of your policy in order to determine what out-of-pocket costs will apply to your treatments, however verification of this information is ultimately your responsibility. Unless you have established an alternate payment agreement with us, all co-payments and estimated co-insurance payments are due at the time of each treatment session; we can assist you in estimating co-insurance amounts. We accept cash, check, debit card or credit card.
In addition to co-payments, deductibles, etc, be aware that health insurance companies now cover little to no equipment suggested by our therapists for use as part of home exercise programs, and the cost of most of these items must now be covered by our patients. This includes, but is not limited to, resistance band or tube, resistance putty, overhead pulley system, pain relief creams and gels, electrodes, etc. Please be aware that some of these items may be suggested and/or necessary for use at home as part of your overall treatment program.
