Coming to Valley Rehab for the first time?
Here's everything you need to know to make your first visit easy.
WHAT TO EXPECT:
All new clients should expect to spend about an hour with the
therapist during their first appointment. During this time, the
therapist will obtain information from you by asking questions and
performing special evaluative tests. This information will be used
to build your customized treatment program, and at the end of your
treatment program the initial information will be used to determine
how much progress you have made.
If you would like to save time during your first appointment and
bring completed paperwork with you, please follow the links below to our intake form and privacy
information. If you are unable to do this, please arrive at
our clinic approx 15 minutes before your scheduled time to complete
this paperwork. If you wish to read the unabridged version of Valley

WHAT TO BRING WITH YOU:

All health insurance
cards that you have

A list of all
prescription and non-prescription medications that you take

A physician’s
prescription if you have one. (If you would like to print our
prescription and take it to your physician, a link is provided
below.)

Shorts (if being treated
for your leg/knee)

Sleeveless shirt (if
being treated for your neck, upper back, shoulder or upper arm)

If your child has a walker, orthotics or braces,
or other mobility equipment, please bring them with
you to the evaluation. What to bring to later appointments will be
discussed during the evaluation.
