Coming to Valley Rehab for the first time?
Here's everything you need to know to make your first visit easy.

WHAT TO EXPECT:

All new clients should expect to spend about an hour with the therapist during their first appointment. During this time, the therapist will obtain information from you by asking questions and performing special evaluative tests. This information will be used to build your customized treatment program, and at the end of your treatment program the initial information will be used to determine how much progress you have made.

If you would like to save time during your first appointment and bring completed paperwork with you, please follow the links below to our intake form and privacy information.  If you are unable to do this, please arrive at our clinic approx 15 minutes before your scheduled time to complete this paperwork. If you wish to read the unabridged version of Valley Rehabilitation’s privacy notice, it is available upon request.
